Breast augmentation in athletic women needs to balance several issues. In thin women, implants may look fake above the muscle, so it may be worth a little extra recovery time for better long term results. On the other hand, under muscle implants may be more prone to animation issues so in these instances I would consider a split muscle technique. Because your breasts are small, and you are accustomed to having some volume, simply removing them when you have the lift is more likely to be disappointing. Your own plastic surgeon is in the best position to advise you.

I have saline implants under the muscle. No problems. It's been 14.5 years. Should I consider replacing them? Is it better to replace before a rupture?

If your implants were placed in the U.S.then they most likely have a lifetime warranty for rupture, and it would be a harmless event. There is no reason to replace them just because they are 14 yers old.

I have lines on my forehead not too bad yet but there is a line between my eyes on top of my nose. Can they be filled if not to deep or is a forehead lift the only option?

Forehead wrinkles are generally caused by hyperactivity of the muscle (frontalis) that raises the eyebrows, and wrinkles between the eyebrows are also caused most often by muscles of expression. Since Botox and similar products like Dysport and Xeomin work by relaxing overactive muscles, they are typically first line treatment rather than fillers. However, keep in mind that the horizontal lines develop because the muscle is working extra to hold up the brow area, which might mean that there is some sagging. In that case, a surgical lift is best because injections might lead to more sagging. See an ASAPS member with expertise in both injectables and browlifting for an expert opinion specific to your situation.

Perhaps a resurfacing laser treatment would be more effective for you that fractionated lasers. A facelift will elevate and reshape the face in ways that skin tightening procedures do not, so your best answer might be both. The laser peel could be done around the mouth and eyes at the same time as the facelift.
